What you really need:
17 mm open end wrench (spanner). Never do this with pliers!
2.5 mm allen key

How you really do it:
Remove the small allen bolt which holds the plastic cover.
Slide the plastic cover up the mirror arm.
Loosen the 17 mm lock nut, if necessary.
Position the mirror where you want it and tighten the lock nut.

If the lock nut does not hold the mirror firmly in place, do the following
Unscrew the mirror and remove it from the headset.
Clean and lubricate the threads.
Screw back into the headset and tighten the lock nut.
Replace the cover.
