Happening at the gear selector. It would be the one that is closest to the tire so it will prevent me from getting into 1st unless I stop and re-tighten. Do these things strip out? Or is there a trick I am missing?

If it's not missing the disc and you're tightening it down tight and it's still coming loose then just replace it. They are cheap and not worth hassle of having to mess with it.
