Looks like the rear brake oil seal has gone on my 2003 px 125.
Two questions .
1) Firstly do I have to split the cases to replace it? if so......
2) This post ( Info: Inner vs outer rear hub oil seal )I can just put in an external seal, is this possible? if so, what seal? is it special?

You don't need to split cases. Just remove felt ring and put seal there.

I done it few times on PX 125 near 2000 year and Cosa 2 200
The external seal for PX was 31*47*7 and 32*47*7 for Cosa.

I also use bearing with rubber(?) seals for example SKF 2RSH.
PX 31*47*7
PX 31*47*7
Cosa 2 200 32*47*7
Cosa 2 200 32*47*7

I found this in my own thread. Cases that have a weep hole require an inner seal and sealed bearing. Otherwise not. Picture courtesy of SaFis.

I prefer to use simple blue Corteco without any extra specs.
But on the other side there is seals what have spin direction.

Here is seal for driveshaft Vespa ET4/LX/S. And you can see marking arrow for spin direction. Also there is notches on the inner lip.
