Recent news of an Alaska airline door plug falling off mid flight. Stop.

Some initially mistook the strange tail art and under belly hook, as signs of a bodge - naturally assuming the bent nail had slipped out. Stop.

Alas, when the plane was found to be manufactured in the US, investigators turned their focus to the possibility that someone forgot to install the cotter pin…

Article says "roller pin" but there is no such thing to my knowledge.

There are "roll pins" and "cotter pins".
But if the bolts are loose - someone didn't remember to put the pin in. I can understand how our crowd of Neanderthals does that - but how did an entire process just fall out of practice up there? (See Safis response I suppose…)

Perhaps NSMVs unofficial tag line should be: don't forget the cotter pin (tho I like saftey third also).

That area is about 20 minutes from me. I recognize that exactly very spot in the pic. I have been past that road hundreds of times on my scooter.

I'm sure we could have designed the door blank better on this forum.
Unlikely Boeing will tell us why it failed. Going to be similar to the nosediving MCAS issue inquiry.

Roll pin might mean an 'R' clip.

EDIT: Just remembered, roll pin is that type of round spring pin that is banged in and punched out. Not the easiest to use

I'm pretty sure that missing cotter/split pins holds the record for most injury-causing accidents on this forum.

I may hold the record for Most Individual Mishaps, but I try to constantly innovate in the field of Industrial Un-Safety. No missing cotter pins for me. Razz emoticon

Anyone new to the forum (less than 5 years) should read this... very entertaining and the basis of many jokes years later.

2nd ride after rebuild...wheel fell off.....went to hospital

Ginch, took your advice and read through this thread. Since I'm in the business of cotter pins and safety wire on everything the OP's attitude pissed me off. Almost couldn't take it till the Challenger discussion. I then proceeded to laugh my tits off till the end!🤣😂🤣 Totally agree that people new to scoots need to read this.

P.S. Upon further reflection due to the inherent dangers of vintage scooters and aircraft, I'll be selling off everything two wheeled and quitting my job. I've decided to take up flower arranging. I now understand the importance of cotter pin thread. Thank You.
