I was talking to Hippo Drones (uk youtuber) on twitter and he told me that the UK's nuclear submarines are metric on one end, imperial in the middle because that part came from America, and then metric on the other end.

were the bolts and nuts suppose to be new 1 time use ?

what was the calibration date on the torque wrench?

in the AF we had to write the s/n of the torque wrench in the aircraft forms for that particular repair item.

it will be interesting to see the detail investigation and root cause findings

from the little bit of reading (on A&P forums) and talking to friends in the field the short answer is that there was no protocol for inspection because it wasn't a "in service" door, so it got left off the check sheet.

the longer answer is somewhere between: the "roll pins" were not spec'd properly and sheared due to design, there was supposed to be a lock nut that wasn't there or wasn't torqued, there was supposed to be a cotter pin that wasn't there because the bolt wasn't drilled for one, the mfgr of the door components was machining on a jig that wasn't spec'd or in compliance with what the engineers had for tolerance, the end user installed the components improperly.

the only thing that everybody agrees on is that it was a systematic breakdown of oversight-- it doesn't matter what and how and it should've been caught on inspection.

I can't say without knowing all the facts. from people I know it seems like a little of column A (engineering), a little of column B (inspection) and a lot of column C (oversight)

engineers are saying it's fine, it just needs inspected more. tech says it shouldn't need to be inspected that often, build it better. parts supplier says don't look at us, it passed QC. mfgr says, that's the only one that's happened to out of [blank], and you guys handle the final assembly *deftly wipes hands*.

the only thing everybody seems to agree on is that the structure didn't fail (ie. the door-- which technically isn't even a door-- didn't buckle), it is something in the attachment. so that why engineering, A&P and mfgr are all standing around doing the Spiderman pointing gif.

It seems like everything from the toaster, to the thermostat and the doorbell can be connected to the web. No thank you, I don't need my info passed or sold off. I'm low tech as I still use beam torque wrenches. I also drive a small truck that has a no chip key and crank windows. Less to go wrong and still running great with regular maintenance for 270K miles. I'm all for useful tech, but not at the expense of my privacy, but I guess it's all out there anyway thanks to a former governor of my state.
