If I have a rear luggage rack, as seen below, can I add the rear back rest in photo on top of the luggage rack? Or do I need to buy the combination back rest and rear luggage rack combo?

There is a seat missing from your rack.

I hope you keep your USA made rack over that low quality Asian rack. Maybe just find the seat for your USA made rack; it is adjustable as well.

I have the same rack on my 200 with no backrest. I read somewhere that they were sold with or without. I would love to have the backrest that goes with it, but have never seen one.

I know it's not really the answer you want but you might just mount a topcase with a backrest pad on it.
I love mine when I'm stuck somewhere waiting, like at a train crossing. Put it on the Center Stand and lean back and relax. (Not to mention having helmet storage when I get to where I'm going.)

That's actually not a bad idea at all! Especially because my seat is blue, and all the 'standard' backrest are black. I could get a colored topcase and have fun with the style. It's all about having fun! Can you share the topcase you have? I've got a px.

I used the one in your second photo, but with a fold down rack. Cuppini I believe. It was a great backrest and very sturdy and well made. However, it was quite awkward getting the side panels/cowls on and off. If you buy one, make sure you get your motor running correctly before fitting, intention being to avoid having to remove the cowl frequently to change jets, check spark plug etc.

Here are some old pictures I had. It's an original Vespa Topcase with a couple of modifications.

Note the top red "light" is only a reflector so I added the lower light. It's a truck side marker LED and I wired it into the brake light. In the second picture you can see the hinge had broken so I replaced it with a piano hinge.
