Is this how the top bearing of het middle headstock should look like?
The balls are vissible, that seems very strange to me. There is a rubber sleef that covered the balls. It is folded back in the picture. Is not very tight fitting and seems to look like a piece of an inner tube from a tyre.Water or dirt could easily get in I presume.

Is this how Piaggio disigned this, or are there parts missing?[/img]

yep this is designed that way, but looks like you have a ball bearing broken, maybe put some new ones in, link below is just an example

Check the ring nuts are done up OK - the bottom one 'just nipped up' and the upper one to lock it.
if there's still play, check the connections to the tie-rod (whatever it's called) that connects the steering column to the suspension columns.

The play is gone after adjusting the nuts.

Is the rubber sleef also standard? Or an "improvement" by a previous owner?
It has a tear in it and I can't seem to find it in the parts drawings.

Yes, back in the 1940s. The MP3 has smaller lower bearings than modern Vespa scooters, and some of the parts are the same ones used on models in the 1950s
