Mongoose posted a tutorial on sewing patches, which looks very nice. It has good pictures and should answer your question.

Take a few tiny stitches through the fabric where you finish off the sewing, looping the needle through the thread each time as you pull tight. It will never come undone.

I put a drop of "Fray Check" on it. It's like invisible glue for sewing. It's about $3 at the fabric store.
