While looking at Motom pictures I found a few really well done custom Lambretta engines. This goes way beyond the typical grafting of another cylinder on to the cases.

First we have a V-twin with two cranks and two chains to the clutch.

Then we have a 4 stroke Lambretta with what looks like a Moto Guzzi Falcone top end on it.

That's an option, even a more reasonable one, but not nearly as unique. I was more stunned by the approach than the twin-ness of it. And the 4-stroke! To have a 500cc 4stroke Lambretta that burned more oil than a stock bike would rock!
